Franciscan Home Care and Hospice Care Introduces New Hospice Care Volunteers

Franciscan Home Care and Hospice Care welcomes newly certified hospice volunteers Gloria Belliveau-Lenihan, Cheshire; Denise Deveau, Bethany; Laura Borrelli, RN, Hospice Director; Chad Donovan, Cheshire; Ken Wargo, Cheshire; and Barbara Blau, Durham.  The volunteers completed 16 hours of training in order to be certified.

SisterAnnouncing Transition to Home Nurse
Franciscan Home Care and Hospice Care announces that Sister Jilda Marie Chabot, FSE, BSN, RN, has assumed the new role of transition to home nurse.  Sister will customize your health care needs when you transition from the hospital or skilled nursing facility to home.  She will coordinate care with your physician, pharmacist, and the hospital to help ensure you have the necessary medications and support for a successful transition to home.  The transition nurse also helps to make sure your home health care needs are being met, therefore reducing your risk of being re-hospitalized. 
Sister Jilda Marie Chabot, a Franciscan Sister of the Eucharist, has been a registered nurse since 1991, and a full-time Franciscan Home Care and Hospice Care nurse since 1997. She is a certified wound specialist. Sister Jilda Marie worked as a medical surgical nurse and as a step down coronary care nurse prior to being a home care and hospice care nurse. She received her Bachelors Degree in nursing from Idaho State University, Pocatello, Idaho.
